Seeking a full time (37.5 hours/week) Clinical Care Manager – Long Term Services and Supports for Elder Services of Worcester Area, Inc.’s contract with Central Community Health Partnership (CCHP) agency. This is a hybrid work scheduled (minimum 2 days per week in the office). The Clinical Care Manager will provide and oversee integrated care coordination and care management to individuals enrolled in the CCHP’s Long Term Services and Supports (LTSS) Community Partners Program. The Clinical Care Manager will supervise a team of Care Coordinators who provide integrated care management in collaboration with individuals served, community partners, medical and behavioral health providers. The Clinical Care Manager will also participate in trainings, meetings, statistics, program procedure development, and other tasks as listed in the job description.
