Responsibilities

The Intensive Home-Based Therapeutic Care program provide services for youth with emotional challenges and/or multistressed families. The scope of services includes flexible individualized treatment, rehabilitation, supervision, and support that vary in intensity, depending on youth and family needs. The services are delivered by a core team of staff using a Wraparound Philosophy support which helps family, support members, and service providers work together to support the family or child. In addition, all services are delivered using a Trauma Informed Care approach.

 

The Outreach Counselor will provide direct support weekly, and ongoing, to each assigned family and youth.  The services will be tailored to meet the unique needs of each youth and family served, providing opportunity to maximize their ability to spend time together at home and in the community. They will also collaborate with the team to promote a culture in which each individual served, and their family member’s point of view, experiences, and preferences are recognized, understood, and respected, and in which self-determination and decision-making in treatment planning are maximized and supported.

 

Other Key Responsibilities:

  • Support the Individual Treatment Plan goals for all youth & families.
  • Assist individuals in identifying individual strengths and interests.
  • Assist individuals in developing the skills needed for self-management, including crisis and prevention planning, social skill enhancement, daily living skill education, stress management, education about mental illness and symptoms and self-reinforcement and contingency management while monitoring their progress toward achieving identified goals.
  • Establish and set appropriate limits and boundaries as a practitioner in order to assure the safety and protection of the family members and maintain the helping role of the practitioner.
  • Facilitate and participate in recreational activities with youth within their family’s community.

Qualifications

  • High School Diploma, GED or equivalent, required.
  • Minimum of two years’ experience working with youth and families required.
  • Valid Driver’s License and acceptable driving history, required.  
  • Reliability, willingness to learn, and being open to new opportunities.
