Responsibilities

The Central Community Health Partnership is a collaboration of five premier human service organizations in Central Massachusetts that has been selected to provide Behavioral Health Community Partner services for people with Behavioral Health Challenges. The partnership helps individuals with complex needs navigate access to services and assist in finding the right combination of services, as it can be an overwhelming task. The vision is integrated care that ensures that every member receives the right care at the right time – making good health and wellness a reality, not a dream.

 

The Clinical Care Manager will provide and oversee integrated care coordination and care management to individuals enrolled in the Central Community Health Partnership’s Behavioral Health Community Partners Program. They will also will supervise a team of Care Coordinators who provide integrated care management in collaboration with individuals served, community partners, medical and behavioral health providers.The Clinical Care Manager will also review and oversee the assessment and person-centered care plans for clinical appropriateness, appropriate service referral/utilization and integration across all health domains.


The schedule for this role is Monday-Friday 830a-430p with a hybrid working schedule of remote and in office.


Other Key Responsibilities:

  • Work collaboratively with the CCHP Behavioral Health team and medical and behavioral health providers to coordinate clinical information for care management, coordination, care transitions, medication reconciliation and health and wellness coaching.
  • Collaborate with Individuals Served, Care Coordinators, Medical and Behavioral Providers, and other members of the team in the implementation and monitoring of Integrated Treatment Plan (ITP).
  • Hire and lead the care coordination/care management staff providing regular supervision, performance appraisal and professional development.
  • Oversee the collection and analysis of outcomes information, annual analysis and other reporting and initiatives as required.

Qualifications

  • Must have one of the following licensures: RN, LICSW, LMHC, LMFT,.LADC I, Licensed Psychologist, LCSW, LRC or Registered Occupational Therapist.
  • Minimum five years experience working with adults with serious and persistent mental illness and substance use disorders, required.
  • Minimum of two years supervisory experience, required.
  • Experience with local resources, navigating medical health, behavioral health and/or substance abuse treatment systems, required.
