The Central Community Health Partnership (CCHP) is a collaboration between Adcare Hospital, Alternatives Unlimited, The Bridge of Central Massachusetts Inc, and Venture Community Services Inc. The Community Partner program is the result of MassHealth’s new model of Accountable Care Organizations (ACO’s) which is in the startup phase across the state. The CCHP team will be comprised of Care Managers and Care Coordinators employed by each partner agency providing support to individuals within the agency’s geographic area and supported by a centralized core team to meet the goal of the CCHP program.
Our goal is to provide care coordination to Individuals with Behavioral Health (BH) and Long-Term Service and Support (LTSS) needs throughout Central MA.
